DIABETES SEMINAR
For Your Information from…Cornell Cooperation Extension of Suffolk County
DO YOU KNOW? According to the American Diabetes Associations, 23.6 million children and adults in the United States - 7.8% of the population - have diabetes.
DIABETES SELF MANAGEMENT SERIES…Cornell Cooperative Extension, in collaboration with the Suffolk County Department of Health Services, is offering diabetes self management classes county wide, beginning Tuesday, March 2nd. These sessions are designed to help people learn to live with and manage their diabetes. There is no charge, however seating is limited – please call to register. See the schedule below.
County Executive Steve Levy added, “Diabetes is a major public health concern in Suffolk County and nationwide, and the rate of the disease continues to rise, especially in the African American and Hispanic populations. This program offers those with diabetes an excellent opportunity to learn more about managing this disease and living life to the fullest.”

HOME ENERGY ASSISTANCE PROGRAM (HEAP)
IF YOU ARE IN NEED OF HEATING ASSITANCE THIS FACT SHEET COULD APPLY TO YOU
Eligibility for HEAP is based on income and household composition. A person’s total, gross monthly income for their household size must be at or below the standards set by the State. For example, for a household size of 1, the monthly income limit is $1,963. For a household size of 4, the monthly income limit is $3,776.
NON-EMERGENCY HEATING AND UTILITY BENEFITS – REGULAR HEAP
To obtain an application for non-emergency situations call 631-853-8825. Facilitated enrollers can assist, and will forward them directly to the Department of Social Services. These facilitated enrollers are:
Regular HEAP benefits are usually set at the following amounts: $800 for non-utility fuel (home delivered heating fuel), such as oil, propane and kerosene; up to $585 for utility heating fuel (piped or wired in), such as natural gas, electricity or wood if it is your primary heat source.
